Vision Home Care is seeking a Registered Nurse (RN) or Licensed Practical Nurse (LPN) to support our growing non-medical home care agency.
This role focuses on client assessments, care plan development, and caregiver supervision—no heavy clinical workload or facility-based stress.
Key Responsibilities
- Conduct initial in-home assessments for new clients
- Complete bi-monthly supervisory visits
- Develop and update care plans based on client needs
- Monitor and document changes in condition
- Communicate with home health aides, families, and case managers
- Ensure compliance with Ohio Medicaid / PASSPORT waiver requirements
- Provide guidance and support to caregiving staff
Qualifications
- Active RN or LPN license in Ohio (required)
- 1+ year nursing experience (home care preferred)
- Strong assessment and documentation skills
- Reliable transportation and willingness to travel locally
- Ability to work independently and manage schedule
